periodontal therapy. Our staff will help you to maximize
the insurance benefits that are available under your policy.

that all our fees are fair and reasonable. All our
fees are at or below the national average for periodontists
published in the American Dental Association's 2003 National
Fee Survey. We are happy to provide you with
detailed information about fees usually charged for periodontal
procedures.

$10.00.
The monthly interest rate varies from 1.45% to 1.75%
and is based upon
the amount charged.

is no interest charge if the account balance is paid
